Issues and decisions log template
A project’s open issues with owners and due dates and the decisions on them in one table. Overdue issues stand out at once.
Excel, 10 KB. No macros, no sign-up.
What is inside
- The “Issues and decisions” sheet: issue, date opened, owner, due date, status, decision, who made it and when, linked work and tickets.
- Open issues past their due date turn red.
- Status is picked from a list: open, decided, cancelled.
- An “Instructions” sheet.
How to use it
- Log an issue as soon as it blocks work, and give it an owner and a due date right away.
- When a decision is made, record it, who made it and when, and change the status.
- Name the plan work or ticket it affects — that shows which date is at risk.
- Start project meetings with the red rows.
How it looks in Etapero
In Etapero open issues with owners, due dates, decisions and linked Jira tickets live in a project’s Issues section, and decisions on key issues show in the project overview. Loading the log from Excel into Etapero is not available yet.
