Risk register template with a 5×5 matrix
A register where each risk’s score and zone work themselves out, and a separate sheet shows how many risks sit in each cell of the matrix.
Excel, 12 KB. No macros, no sign-up.
What is inside
- The “Risk register” sheet: the risk as “cause → event → consequence”, category, probability, impact, score, zone, owner, response, review date, status and linked work.
- Score is probability × impact; the zone is red from 15, amber from 8, green otherwise.
- The “Matrix” sheet: the number of risks in each of the 25 cells.
- An “Instructions” sheet.
How to use it
- Describe a risk in one sentence: what may happen, why, and what it leads to.
- Rate probability and impact with whole numbers from 1 to 5 — the score and zone appear by themselves.
- Give every red and amber risk an owner and a response.
- Review the register at regular project meetings and close risks that no longer apply.
How it looks in Etapero
In Etapero the register of risks, problems and assumptions lives in a project’s Risks section, the matrix is built automatically, and red-zone risks show in the project overview and the portfolio. Loading a register from Excel into Etapero is not available yet.
